Samantha Marie DyAnne Hawk was born April 12, 1993 and passed away November 18, 2015 from complications of Rett Syndrome.

She took her last breathe in the arms of her mother, surrounded in love by family and friends, both present and in spirit.

Services for Samantha will be held at Omega Funeral & Cremation Service, 223 NE 122nd Avenue, Portland, Oregon December 1st at 2 PM with a reception to follow.

She is survived by Mom & Dad, Tracie and Michael Hawk, Sister Micheal Hawk, Brothers Adam Hart, Anthony Hawk & Jaycob Johnson, Grandma Donna Green, her Aunt Kelli Green, Uncle Jason Green, cousins Dyln & Seth Vickers. Also numerous nieces and nephews and extended family.

Samantha spent several years in the loving care of Vicki Girrardin and Norma Ablack Manning.
In Samantha’s life, no one has ever walked away from her without being impacted by her in some way. Her infectious smile and laughter, her intent stubbornness, her strong amazing will to survive and fight, to the wonder and depth of her soul through her gaze.

Whether you have just met her or have known her for a lifetime, she will become a part of you.
Samantha’s legacy will live on, through the heart and soul of many.
